Scaling up interventions to improve the control of hypertension and diabetes in partnership with the governments of Kerala’s and Tamil Nadu. "Leveraging India’s national NCD program"
This Scale-Up intervention is a five-year project (2019-2023) that draws on evidence-based approaches to noncommunicable diseases (NCDs) in India to better link and integrate prevention with disease management at both a community and systems level. This project aims to design and deliver a scalable Structural Lifestyle Modification program adapted from previous NCD trials in India for rollout across the states of Kerala and Tamil Nadu in India. The intervention model consists of an integrated screening and lifestyle modification program, which includes peer group and community mobilization components and technology enablement, to increase awareness of cardiovascular disease control, to provide support for individuals with diabetes and/or hypertension and for individuals with moderate to high levels of cardiometabolic risk. The model leverages India’s Community Health Workers and Accredited Social Health Activists to deliver the programs in their communities. Developing the program will involve a co-design process with health services and community stakeholders in both states and ongoing collaboration with State NCD managers to integrate the program into the National Programme for Prevention and Control of Cancer, Diabetes, Cardiovascular Disease, and Stroke.
